About the practitioners

Gusti Labatte-Deneau, APRN, ANP-BC, ACHPN, CDP, PMHNP-C

Gusti Labatte-Deneau is a graduate of City College of New York where she obtained her bachelor in nursing in 1992. She worked at Lenox Hill Hospital for 10 years where she gained experience in medical-surgical, intensive care and cardiothoracic surgery. There she worked many roles: charge nurse, supervisor, preceptor. She later obtained her master’s degree at Hunter Bellevue School of Nursing in 2005 with a certification in Adult Health. She then joined the prestigious Gyn-Med-Onc team at Memorial Sloan Kettering Cancer Center where she worked for three years managing patients on the inpatient unit prior to moving to Florida in 2009 where she spearheaded and started the innovative palliative care program with Treasure Coast Hospice. Being the only nurse practitioner on the team, she precepted and trained other nurse practitioners and grew the service to 4 full-time APRNs. She left Treasure Coast Hospice to attend school, worked Per diem while starting her mobile practice making housecall visits to homebound patients on the Treasure Coast and those residing in assisted living facilities. In December 2018 she graduated as a foreign medical graduate from USAT at the island of Montserrat. She opened an office practice in Lakewood Park, Fort Pierce when covid struck servicing local and home patients. She continues to be an integral part precepting the next generation of nurse practitioners, all while attending Liberty University completing a post mental health nurse practitioner certificate. In the summer of 2024, she completed her post master’s mental health nurse practitioner certificate at Liberty University and passed her certification on in December of 2024. She received her active certification in mental health from AANP in February of 2025. She now plans on incorporating this much needed layer into her practice remotely. She offers concierge service options for those who desire with a monthly fee.


She transitioned from office to now focusing her practice solely making housecall visits to homebound patients and those residing in Assisted Living Facilities. She is an active vital member of the community and volunteers with various organizations as well as participate in the advisory Council for Alzheimer’s community care. She

takes part in medical missions overseas providing care to the forgotten and underserved populations. She is an associate Jewel of the Treasure Coast chapter of Jack and Jill. She volunteers on the EMS team at church and served on the conference planning committee of GAPNA (Gerontological Advanced Practice Nurses Association) and the NPCTC (NP council of the Treasure Coast) and is a mentor for the Leroy Jefferson Medical Society.


When not working, she can be found partaking in social events enjoying spending time with her family. She is the proud mother of 24-year-old twins (a boy and a girl) and 20-year-old son and is married to her husband of 32 years, now retired Fort Pierce Police Officer. She loves music, going out with friends, traveling, discovering new hidden treasures in Florida, reading, watching action movies. She loves her two dogs.


She is fluent in French, Creole, speaks limited Spanish and currently learning Italian.


Her practice has been chosen to partake in the national home-based primary care learning network project 2024-2025 beginning June 2024; the only participant from the state of Florida, dedicated to improve the quality of health of homebound patients. Once completed, Exceptional Care Choice will join to be the 5th practice in the entire state for that stature.

Leona Sandelier, DNP, APRN, FNP-C, is a doctoral-prepared, board-certified Family Nurse Practitioner with experience spanning primary care, nephrology, pediatrics, critical care, and post-acute care. She is passionate about delivering patient-centered, evidence-based care with a focus on building meaningful relationships and improving long-term health outcomes. Leona’s background includes managing complex acute and chronic conditions across a variety of healthcare settings, including oncology critical care and nephrology. She is dedicated to providing compassionate, accessible care while empowering patients to take an active role in their health and wellness journey.
