Welcome to Exceptional Care Choice, LLC! Where quality in-home healthcare is our focus. We offer skilled nursing, physical therapy, and referrals to specialists. We accept Traditional Medicare and PPO insurance plans. Flexible payment options include credit cards for easy transactions.
This mobile practice is independently owned and operated by Gusti Labatte-Deneau, autonomous APRN, board certified as Adult Nurse Practitioner ANP-BC, as well as Advanced Certified Hospice and Palliative Nurse, ACHPN and Certified Dementia Practitioner, CDP, providing timely personalized housecalls and medical services in the comfort of your home. Treatments will be coordinated with other specialists previously established with patients. When appropriate, we arrange for diagnostic imaging/testing, laboratory studies, refill medications, initiate home health services when needed, order durable medical equipment, complete 1823, 3008 forms and other at- home services to ensure that all your needs are met.
